The International Society of Minimally Invasive and Virtual Surgery (ISMIVS, previously known as International Society of Minimally Invasive and Noninvasive Medicine) was founded in July 2013 at the gala of the 1st Yangtze International Summit of Minimally Invasive and Noninvasive Medicine in Chongqing, China. ISMIVS envisions to accelerating the progress in minimally invasive and noninvasive medicine by promoting research, education, communication, and international collaboration.

The Personality in the Medical Education World 医学教育界的大人物
From the 2020 July issue of our ISMIVS Newsletter, we would like to introduce some pioneers in the field of minimally-invasive and noninvasive medicine. They are not only prestigious in their field but also willing to share their experience and ideas with younger doctors, hence making a great contribution to medical education. This issue features Dr Jordi Rodríguez González from Barcelona, who is engaged in the field of high intensity focused ultrasound (HIFU).

Dr Jordi Rodríguez González graduated in Medicine and Surgery from the Barcelona Central University (UB) in 1997, then specialized in Obstetrics and Gynecology at the Mútua Terrassa University Hospital, Barcelona, Spain from 2001 to 2005.

He was trained in the HIFU Training Base of China in 2009 and obtained certification in HIFU training in adenomyosis. With over 12 years of experience in HIFU clinical application, he is now the Head of HIFU Unit, Department of Obstetrics and Gynecology, in Mútua Terrassa University Hospital, treating nearly 1000 cases of gynecological disorders.

To continue the fascinating serial sessions of 2021 and also as a response to the cordial call from the global audience, the International Society of Minimally Invasive and Virtual Surgery (ISMIVS) and Asia-Pacific Association for Gynecologic Endoscopy and Minimally Invasive Therapy (APAGE) initiate 2022 HIFU Series Webinars (AIH 2022) in May.

AIH 2022 upgrades the program with hybrid session themes, which focuses on different gynecological diseases in 4 different episode, aiming to promote women’s health and preserve fertility through up-to-date scientific research, clinical practice and live demonstration from a line-up of world renowned physicians in the field of minimally invasive and noninvasive medicine.

The 1st episode of AIH 2022 “Management of endometriosis, especially for patients with fertility desire” kicked off on May 19th, with Dr Olarik Musigavong from Thailand and Dr Relly Yanuari Primariawan from Indonesia being the Session Chairs. Nearly 2000 audiences from 56 countries and regions joined this session online, making a strong start for AIH 2022.

Prof. Chyi-Long Lee, the Chairman of the Board of Trustees of APAGE, discussed the therapy for endometriosis in reproductive women. He first indicated that endometriosis was found in 10% of women in reproductive age and about 176 million women suffer from the disease worldwide, then he explained the mechanisms of mild or minimal endometriosis and infertility based on the paper and how deep endometriosis impacts fertility, based on which he introduced several surgical techniques including “A to Z” of DIE treatment. At the end of his lecture, he emphasized that laparoscopy should remain the gold standard whenever DE surgery is indicated and continuous training would always be the last puzzle for reproductive surgery.

Prof. Lian Zhang, Secretary-General of ISMIVS, Director of Chongqing Haifu Hospital, compared ultrasound-guided HIFU (Usg-HIFU) and other surgical options for abdominal wall endometriosis (AWE). He first introduced the principle and indications of HIFU, showing some contrast enhanced MRIs of a patient with AWE before and after HIFU treatment. In addition, he indicated that there was no skin burn encountered, the pain did not recur during a mean follow-up of 18.7 months, and all treated nodules gradually shrank over time, then concluded that HIFU treatment was promising and could be used to treat AWE safely and the effectiveness of HIFU treatment for AWE was comparable with open surgery, with less adverse effects and short hospital stay.

Dr Min He, the Senior Training Lecturer, shared the learning curve of HIFU and her training experience in China, she indicated that basic requirements for clinicians to learn HIFU included specific medicine majors (ob-gyn, oncology, surgery, breast surgery, orthopaedics, medical imaging), image recognition ability (US/MRI), and understanding of HIFU. She also mentioned that factors affecting the learning curve of HIFU include ultrasonic image recognition ability, clinical experience, HIFU treatment related knowledge, number of cases and operation frequency. Since 2005, 2150 professionals have been trained in China’s HIFU training center.
HIFU 高级培训讲师何敏医生则分享了 HIFU 的学习曲线和她在中国的培训经验,她表示学习 HIFU 的临床医生需毕业于特定的医学专业(妇产科、肿瘤科、外科、乳腺外科、骨科、医学影像)、具备图像识别能力(US/MRI)和对 HIFU 的理解能力。她还提到,影响 HIFU 学习曲线的因素包括超声图像识别能力、临床经验、HIFU 治疗相关知识、病例数和手术频率。自 2005 年以来,中国的HIFU培训中心已经培训了 2150 名专业人员。

Episode 2: Treatment for Placenta Accreta and Cesarean Scar Pregnancy
第 2 期:胎盘植入与瘢痕妊娠的治疗
Themed “Treatment for Placenta Accreta and Cesarean Scar Pregnancy”, the 2nd episode of AIH 2022 opened on June 16th. Episode 2 invited Prof. Tsutsumi Osamu (Director of Sanno Hospital, Japan) and Prof. Chi-Chang Chang (President of Taiwan Association for Minimally Invasive Gynecology) to be the chairs, with 3 leading experts discussing minimally invasive and noninvasive therapies for patients with placenta accrete and cesarean scar pregnancy.
6 月 16 日,AIH 2022 第二期研讨会“胎盘植入与瘢痕妊娠的治疗”正式开讲。本期研讨会邀请到 Tsutsumi Osamu 教授(日本山王医院院长)和张基昌教授(台湾微创妇科协会会长)担任主席,联合 3 位国际权威专家探讨了针对胎盘植入与瘢痕妊娠患者的微无创治疗方法。
Prof. Aizura Adlan, Associate Professor of Department Obstetrics and Gynecology, University of Malaya, discussed the pathogenesis, diagnosis and management of placenta accreta and cesarean scar pregnancy (CSP). She believed that conservative management should be considered only for carefully selected cases of placenta accrete spectrum, cases that be considered investigational and after detailed counselling about the risks, uncertain benefits and efficacy. After demonstrating 4 cases of CSP, she provided several treatment modalities including laparoscopic caesarean scar resection, TAS-guided hysteroscopic curettage, and repeated HIFU combined with hysteroscopic S&C.

Dr Lee Jae Seong shared 13 years of clinical experience in the HIFU treatment for gynecological disorders at a single center in South Korea. From February 2010 to October 2017, his center managed 1807 cases (918 cases of uterine fibroids and 889 cases of adenomyosis), among which the uterine fibroid volume (mean ± SD) before HIFU is 176.46 ± 149.87, and 3 months and 12 months after HIFU is 79.87 ± 74.81 (55%) and 46.23 ± 54.7 (74%). He also provided clinical evidence for the safety and efficacy of HIFU therapy with several research papers, then end up summarizing that HIFU clinicians should be careful with patients who want future pregnancies and inform some possibilities of being in trouble of pregnancy and enough experience is needed to make satisfactory treatment outcomes.
李在星医生分享了他 13 年以来在韩国单中心应用HIFU治疗妇科疾病的临床经验。2010 年 2 月至 2017 年 10 月,他的中心共治疗了 1807 例妇科疾病患者(918 例子宫肌瘤和 889 例子宫腺肌症),其中 HIFU 治疗前的子宫肌瘤体积(平均值±标准差)为176.46±149.87,HIFU治疗后 3 个月和 12 个月分别为79.87±74.81(55%)和46.23±54.7(74%)。随后,他通过多篇研究论文为 HIFU 治疗的安全性和有效性提供了临床证据,并总结道,HIFU 临床医生应谨慎应对有生育需求的病人,告知病症对后期怀孕的影响,同时医生也需要积累足够的临床经验,才能获得满意的治疗效果。
Dr Zhang Cai, the Senior HIFU doctor, discussed clinical protocol and outcomes of HIFU followed by dilation and curettage to treat CSP. She first introduced the types of CSP based on the 2016 Chinese Expert Opinion and indicated that the aim of the management of CSP was to preserve fertility, and prevent life-threatening complications such as massive haemorrhage and uterine rupture. She further shared different sonication strategies of HIFU treatment for CSP and compared the adverse events, efficacy and fertility outcomes of HIFU and UAE, and concluded that HIFU therapy featured non-invasive and real-time monitoring qualities which could reduce the risk of vaginal massive bleeding, minimize damage to the uterine, and keep the fertility.
HIFU 高级医师张彩医师则探讨了 HIFU 治疗 CSP 的临床方案和结果。她首先根据《2016 年专家意见》介绍了三类CSP 分型,并指出治疗 CSP 的主要目的是保护生育能力,并且防止大量出血和子宫破裂等危及生命的并发症。她进一步分享了 HIFU 治疗 CSP 的不同超声策略,并比较了 HIFU 和 UAE 治疗 CSP 的不良事件、疗效和生育结果,认为 HIFU 治疗具有无创和实时监测的特点,可以降低阴道大量出血的风险,最大限度地减少对子宫的损伤,保持生育能力。

The 3rd International Training Workshop on Focused Ultrasound Therapy For Tumors
"High Intensity Focused Ultrasound (HIFU) Therapy"
第 3 届聚焦超声无创治疗肿瘤技术国际培训班
International Training Workshop on Focused Ultrasound Therapy for Tumors, a Chinese government sponsored medical training program, provides training courses on High Intensity Focused Ultrasound (HIFU) therapy for clinicians including obstetricians and gynecologists, oncologists, surgeons, interventional radiologists, etc. Since 2012, under the auspices of the Department of International Cooperation of the Ministry of Science and Technology of the People's Republic of China, it has been implemented for 7 sessions by Chongqing Medical University and ISMIVS jointly with different organizations, having trained 149 doctors from 42 countries. In 2018, this Workshop for the first time went abroad to Cairo, Egypt and continued its overseas education in Johannesburg, South Africa in 2019.
聚焦超声治疗肿瘤技术国际培训班是由中国政府出资启动的医疗培训项目,为妇产科、肿瘤科、外科、介入放射科等临床医生提供高强度聚焦超声(HIFU)治疗技术的培训课程。自 2012 年起,在中华人民共和国科技部国际合作司的支持下,重庆医科大学与国际微无创医学会联合各大组织机构成功开展 7 期培训项目,共计培养来自 42 个国家的 149 名医生。2018 年,该培训项目首次走向世界,于埃及开罗开办,并于 2019 年在南非约翰内斯堡继续开展海外培训。
The 2020 workshop which was planned to be held in Pleven, Bulgaria was postponed due to the COVID-19 Pandemic, now as the pandemic is better controlled, we plan to organize the 2022 Training Workshop in Malacca, Malaysia.

HIFU Special Issue 高强度聚焦超声特刊

We are delighted to announce that the International Journal of Hyperthermia, the HIFU Special Issue (i.e. Volume 38, 2021), which was partially supported by ISMIVS, was officially released in September. The Special Issue includes 15 articles written and reviewed by world-renowned scholars and pioneers in minimally invasive and noninvasive medicine, elaborating on the status and prospects of the clinical applications of high intensity focused ultrasound on tumours that occurred in specific organs. We'll share the 15 articles included in the upcoming newsletters, stay tuned!
Efficacy and safety of magnetic resonance-guided focused ultrasound treatment for refractory chronic pain of medial knee osteoarthritis
Totally twenty patients with medial knee OA eligible for total knee arthroplasty were included in this perspective. MRgFUS treatment was provided at the site of most severe tenderness around the medial femorotibial joint of each patient under real-time monitoring of temperature.
本次前瞻性研究共纳入 20 名符合全膝关节置换术条件的膝关节内侧 OA 患者。 在实时监测温度的情况下,在每位患者的内侧股胫关节周围最严重的压痛部位提供 MRgFUS 治疗。
A 50% or greater decrease in NRS score was seen in 14 patients at 12 months post-treatment. The mean NRS score rapidly decreased 1 month after treatment and continued to decline through the following 12 months. No serious adverse events were observed during and after treatment.

MRgFUS treatments were effective not only for managing refractory pain but also for improving physical functions without adverse events in elderly patients with medial knee OA.

A new fertility-preserving treatment modality for life-threatening bleeding caused by acquired uterine arteriovenous malformation (AVM): Combination laparoscopic ligation of uterine arteries and AVM suture
In this article, the author shared a new fertility-preserving treatment modality for life-threatening bleeding caused by an acquired uterine arteriovenous malformation (AVM): A combination of laparoscopic ligation of uterine arteries and AVM suture.

A 39-year-old female was successfully treated using laparoscopy bilateral uterine arteries ligation followed by application of uterine AVM suture with an absorbable barbed wound closure device. Complete regression of the AVM lesion on sonography was noted 8 months after laparoscopic surgery. Besides, this patient had normal menstruation after the operation.

This case report describes for the first time a successful combination of bilateral uterine artery ligation and AVM sutures to treat a patient with uterine arteriovenous malformation. They demonstrated the efficacy and safety of this fertility preserving method. And the disadvantages of this procedure were the risk associated with general anaesthesia and the skills needed to perform laparoscopic surgery.
